SETTLEMENT CLASS MEMBER ELECTION FORM

For the Class Members, payment will be sent automatically by First-Class U.S. Mail to the last known mailing address.

Class Members also have the option, but are not required, to complete this Election Form to choose one or more of the following options: (a) provide an updated address for sending a check; (b) elect to receive the Settlement Benefit as a Digital Payment such as Venmo or PayPal instead of a paper check.
